NEXT LEVEL DANCE AND CHEER ACADEMY Liability Waiver Form and payment/fees 

I/We understand that Next Level Dance & Cheer operates on a monthly membership of $95, and that a one-time registration fee is due at the time of enrollment.

I/We understand that monthly dues are automatically processed on the 1st of each month, and that a $5 late fee will be applied if payment is not received by the 3rd of the month.

I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Next Level Dance And Cheer Academy. I/we agree to release and hold harmless Next Level Dance And Cheer Academy including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future.i/we agree and understand the 30 day cancellation policy the studio has and understand I/we are responsible for any balances owed if notification is not received. I/we will not hold Next Level Dance And Cheer Academy liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Next Level Dance And Cheer Academy. I understand that Next Level Dance And Cheer Academy is a licensed and insured business. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Owner, Director, Instructor or Staff member as soon as possible.
